Please click the download button at the top of the PDF (between the print and bookmark buttons). On a smartphone or tablet, tap the image of the form, and then you will see the download button. Then, next to "Save As...", rename the file with your name. Open Acrobat Reader, fill out and sign the form, and save again. (If Acrobat Reader is not installed on your computer, click the download button below and follow the instructions.)

 

Attach your  PDF to an email and send it to HandSpring Bodywork (the email address is at the bottom of page 2 of the PDF).
